LIC Policy has to be submitted at the same branch from the place it had been purchased. LIC life Insurance premium can be paid from any part of India but for services like change of nominee, policy loan, any correction in name, correction in date of birth, policy maturity submission has to be done at the particular branch only.
The documents required along with LIC Maturity forms are :
- Original LIC policy certificate,
- PAN Card copy
- One cancelled cheque with policyholder’s name embossed on it.
With these documents, the below-given form is also required to send for getting LIC policy maturity amount directly to the policyholder’s Bank account.
With all these documents and above given filled form has to be submitted to the servicing branch by hand. Don’t forget to write the policy holder’s contact number wherever required.
